SUMMARY OF POSITION
The ECM Program Director will work closely with organization leadership to help lead and develop and manage the Enhanced Care Management (ECM) program. The director is responsible for managing the daily operations of the department, providing leadership and guidance to the staff, and ensuring that all policies and procedures are followed. They will work closely with other internal and external stakeholders to provide seamless care to clients and ensure that all services are delivered in an efficient and effective manner.
Works in collaboration with: Care coordinators, care navigators, case managers, medical/health care providers (primary care physicians, nursing staff, mental health works), community support organizations
