To assist the patient and his/her family or members of the medical health care team in meeting post-hospital health needs through appropriate teaching, psychosocial evaluation and support and referral to community agencies that can provide procedures and treatment measures to restore the patient to his/her optimal physical and mental health. Minimum Education: Master of Social Work or LCSW Preferred. Minimum Experience/ Knowledge: Working knowledge of local, regional and federal health, social, and welfare resources. Required License/ Certification: Current BLS certification from the American Heart Association.
