TCC Family Health was founded in 1939 by a group of physicians and community leaders who recognized the importance of access to health care for all children, particularly those who are from low-income families and who are at risk for health problems. Job Summary:
The Bachelor of Social Work (BSW) is responsible for providing and coordinating behavioral health services for patients as part of an integrative model of care within the primary care setting. The BSW collaborates with and assists primary care and behavioral health staff as well as other internal and external service providers in order to effectively manage mental health and substance use conditions and support physical health outcomes. The BSW coordinates behavioral health referrals, screens patients using structured instruments, educates patients about mental health and substance use conditions and treatment options, and monitors symptoms and response to services.
Under the direct supervision of the Director of Behavioral Health, this position supports the TCC clinical care team, including LCSWs, LMFTs, AMFTs, ACSWs, Clinic Managers and Primary Care Providers and other staff. Independent judgment is required to plan, prioritize and organize diversified caseload with professionalism and discretion. The position continually requires demonstrated poise, tact and diplomacy in working with a diverse group of internal and external partners.
