Position Details

  • Full-Time
  • Location: North Hills
  • Shift A: Sunday through Thursday
  • Shift B: Tuesday through Saturday

Key Responsibilities

  • With input from other team members, responsible for developing, integrating, coordinating and monitoring each client's active treatment
  • Write and supervise the development and delivery of each individual service plan, assure that all residents have Individual Service Plans that have clear and quantifiable objectives
  • Initiate periodic reviews of each individual including the six month and annual reviews of the ISP
  • Conduct or coordinate client assessments and record each resident's progress in client's records
  • Assure consistent and ongoing review of all ISP's assuring appropriate data collection; assure documentation of progress on a consistent basis
  • Supervise the delivery of training and habilitation services
  • Conduct quarterly Human Rights meetings. Assure that all aspects of the program conform to Department of Health Services and Medi-Cal standards
  • As part of the management team, assist Program Director with staff recruitment, training, staff supervision and evaluation. Provide guidance, any necessary training or briefing, and supervision of OT, PT and/or RT consultants and volunteers involved in planning or leading client activities
  • Ensure adequate and appropriate staff orientation, new staff training, and on-going in-service training
  • Facilitate ongoing staff meetings, in conjunction with the Program Director and Program Nurse
  • Review referral packets of prospective residents and facilitate interdisciplinary team meetings to discuss appropriateness of placement in the program
  • Ensure that all resident activity calendars are developed and completed appropriately in a timely manner, making certain that the activities are appropriate for the residents in relation to individual goals, budgetary considerations and program guidelines and that calendars are displayed appropriately in the homes. Assist with the facilitation of weekly program activities for residents, assist staff with the planning and execution of activity groups
  • Assure that all necessary equipment and supplies are procured for activities and treatment planning
  • Serve as liaison with community groups and individuals to provide information to the community as required, including attending community meetings, resource fairs and marketing events to increase community awareness of the program. Engage in community outreach efforts to fill open beds
  • Maintain on-call status in rotation with the Program Director
  • Provide direct care coverage on an emergency basis
  • Provide program leadership in the absence of the Program Director

Qualifications

  • A human services professional possessing at least a bachelor's degree in a human service field, including but not limited to sociology, special education, psychology, rehabilitation counseling and nursing
  • At least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities
  • Administrative and supervisory experience desired
  • Possess a valid CA driver's license with good driving record

The salary range represents the low and high end of the salary someone in this role may earn as an employee of The Help Group. Salaries will vary based on various factors, including but not limited to, professional and academic experience, certification, training, responsibilities of the position, and other business and organizational needs.
