Horizon San Gabriel Valley is seeking a compassionate and organized Admissions Coordinator to play a vital role in our Recuperative Care program. This position is ideal for someone who enjoys working with people, coordinating details, and ensuring a smooth, supportive admissions experience for patients transitioning into care.

Position Overview

The Admissions Coordinator serves as a key liaison between prospective patients and referring organizations. You will guide individuals through the admissions process, ensure all required documentation is complete, and provide hands-on support to facilitate a positive and seamless entry into Recuperative Care.

Key Responsibilities

• Act as the primary point of contact for prospective patients and referral partners

• Coordinate and manage the admissions process from referral to acceptance

• Ensure all required documentation is completed accurately and on time

• Communicate clearly with hospitals, social workers, and referral organizations

• Provide compassionate guidance and support to patients during intake

• Maintain organized and confidential patient records

• Collaborate with internal teams to ensure smooth admissions and transitions

Qualifications

• Prior experience in admissions, healthcare coordination, case management, or related role preferred

• Strong communication and interpersonal skills

• Excellent organizational and documentation skills

• Ability to work independently and manage multiple priorities

• Compassionate, professional, and patient-focused approach

• Experience in recuperative care, healthcare, or social services a plus
